Home Sleep Tests

• Home sleep tests can be purchased for $200 without a sleep consultation.
• Fill out the form below and our sleep coordinator will contact you to set up an appointment.
• You will come to our office to pick up the equipment and our sleep coordinator will provide instructions on how to conduct the home sleep test.
• Return the equipment the following day.
• Results will be emailed to you once they have been completed.
